1.Go to App manager and search for Email Template.
2.Go to Email Template and Click on New as shown in below image.

3.Enter all the information as shown below.
Email Template Name : Enter Email Templatae Name as pe rrequirement.
Record Entity Type : Select the source object.
Description ( optional ) : Enter the description of your email template.
Subject : Subject of the template.
HTML Value : Paste the below HTML code in the body and click on save.
